    1. Open Studios Application Agreement - Due by end of day Wednesday, June 25, 2025. NO late entries.
    Please fill out and complete all required information below. Red asterisk indicates required information. If you do not check all required boxes, your form will not be received, and you will NOT receive a confirmation message.
    ​​In order for CoCA Open Studios to work well for all, we have thoughtfully developed the following guidelines for participants. Please check each box to signify that you read and understood each guideline. Your completed and emailed submission confirms that you are willing to comply with all rules and regulations. 
    Please note: We are not accepting late entries, i.e. after Wednesday, June 25, 2025.

    You must click the SUBMIT button below to send this Application—Don't forget to return to this form to pay your fee and submit your image. 

    Patt will reply with confirmation of receipt of your application along with a link to required tasks. If you do not receive her email, by the following day,     contact her.

​   Please mail your check by Friday, June 20 to make sure it arrives by Wednesday, June 25, 2025.

​   Please note: look at your image actual size (1.25" x 1.25") to determine how it will look in the brochure. 
    Send NEW square jpeg, 900 x 900 pixels or 3" x 3” to: [email protected].  

    Save your image as your first name and last initial (example: PattS)
​    Please put “Your Name-CoCA OS 25” in the Subject and your Name/Medium(s) in the Message.

    When design is complete, you will receive a link via email from Ellen to proof your information on Brochure, Postcards, Flyer, Passport, and Website.
    Please respond with either your approval, or edits by date requested on proof or forfeit your opportunity to make edits.
